We understand that your health information is personal. We are committed to protecting your health information and complying with all applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 regulations governing the confidentiality of substance use disorder patient records.

Special Protections for Substance Use Disorder Treatment Records

As a substance use disorder treatment facility, your records receive additional federal protections under 42 CFR Part 2. These regulations generally prohibit us from disclosing information identifying you as having or having had a substance use disorder, or from disclosing any information from your treatment record, except:

  • With your written consent
  • To medical personnel in a medical emergency
  • To qualified personnel for research, audit, or program evaluation
  • When authorized by court order
  • When required by state law for reporting suspected child abuse or neglect

Your Rights Regarding Your Health Information

You have the right to:

  • Get a copy of your health and claims records
  • Correct your health and claims records
  • Request confidential communication
  • Ask us to limit the information we share
  • Get a list of those with whom we've shared your information
  • Get a copy of this privacy notice
  • Choose someone to act for you
  • File a complaint if you believe your privacy rights have been violated

How We May Use and Share Your Information

For Treatment

With your written consent, we may share your information with treatment team members and other healthcare providers involved in your care.

For Payment

With appropriate authorization, we may share information with your health insurer to obtain payment for services.

For Healthcare Operations

We may use your information for quality improvement, training, and other healthcare operations with appropriate safeguards.

How We Protect Your Information

We maintain physical, electronic, and procedural safeguards including:

  • Secure electronic health records with encryption
  • Staff training on privacy and confidentiality
  • Physical security measures for records
  • Audit trails for access to electronic records
  • Business associate agreements with vendors
